National Cancer Plan 2022-2027

Obesity in adults: Clinical practice guideline adapted for Chile

The Chilean Society of Bariatric and Metabolic Surgery, together with other scientific societies, led a process for adapting the Canadian clinical practice guideline for obesity in adults for Chile. The aim of the Canadian guideline, among its main objectives, was to propose changes in obesity management using a chronic disease framework and focusing on improving patient-centered health outcomes, rather than focusing on weight loss alone.

National Health Strategy 2021-2030

The National Health Strategy sets Chile's Health Objectives for 2021-2030, with the ultimate goal of improving the health and wellbeing of the entire population. Two of its key axes are lifestyles and chronic non-communicable diseases, with goals to increase access to and availability of food for a healthy diet, promote and improve access to regular physical activity, reduce the prevalence of obesity and overweight, and reduce the burden of non-communicable diseases including cardiovascular and cerebrovascular diseases, diabetes mellitus, chronic kidney disease, and cancer.

Physical activity and exercise according to life course and comorbidity

This document provides physical activity guidelines for pregnant women, children, adolescents, and adults, as well as people with a history of medical conditions including high blood pressure, type 2 diabetes mellitus, dyslipidaemia, overweight and obesity, and chronic obstructive pulmonary disease. "Hoy empiezo a vivir sano" (Today I will start to live healthy)

“Hoy empiezo a vivir sano” (Today I will start to live healthy) is the slogan of a 2020 communications campaign launched by Elige Vivir Sano (Choose to live healthy). The campaign aired on mainstream media and social networks. It aimed to motivate people to make small changes in their routines to adopt healthier habits and combat excess weight and obesity, conditions that currently affect seven out of ten people over 15 years old in Chile. The actions being promoted include cooking as a family, prioritizing fruits as a snack, going for a walk, playing with children. These actions are easy to carry out and can have a significant impact on the quality of family life.

An evaluation of Chile’s Law of Food Labeling and Advertising

This study evaluated the effect of Chile's 2016 Law of Food Labelling and Advertising (Ley 20.606) on the purchase of sugar-sweetened beverages. This law mandated front-of-package warning labels, bans on child-directed advertising, and sales in school of all foods and beverages exceeding established limits on sugar, salt, saturated fat or calories. The researchers found that following the regulation's implementation, "the purchase volume of high-in beverages decreased by 22.8 mL per capita per day or 23.7%".

Measures for the Protection of Breastfeeding (Ley 21.155)

This law establishes protection measures for breastfeeding and its exercise. It recognises the right to breastfeeding as a right of children, enshrines the right of children to access human milk and the right of mothers to freely breastfeed their children, and guarantees the free exercise of breastfeeding, sanctioning any discrimination that causes deprivation, disturbance or threat to these rights This means that mothers are not required to conceal their breastfeeding or use special breastfeeding rooms, but these rooms must provide safe, comfortable, and hygienic conditions. Employers must also provide mothers with the facilities to express and store their milk.

The Law on Food Advertising (Ley 20.869)

This law prohibits any kind of television or movie advertising between 06:00 am and 10:00 pm for foods or beverages that exceed cut-off points energy, sugar, saturated fats and sodium determined by the Minister of Health. The law also bans any kind of advertising of breast milk substitutes.

National Food and Nutrition Policy (PAN)

The Food and Nutrition Policy establishes two guiding principles: 1) people's right to food as an unavoidable ethical principle when developing programmes, initiatives and regulations; and 2) food and nutrition are socially determined, and the conditions in which people's lives unfold shape food consumption decisions. The policy seeks to build healthier food environments, improve access to nutritious and sufficient food, promote healthy eating, and reduce the prevalence of overweight and obesity.

Using evidence-informed policies to tackleoverweight and obesity in Chile

Overweight and obesity are a global epidemic with rates having risen to alarming levels in both developed and developing countries. Chile has been no exemption, with sharp increases in obesity prevalence, especially among school-age children. This paper describes the policy actions and strategies implemented to tackle this major public health concern in Chile over the last 10 years, and highlights the main challenges and nuances of the process. Chile has taken policy action that includes front-of-package labelling, advertising regulations, and school-food restrictions. New policies focus on the social determinants of health as they relate to food environments and people’s behavior. These actions are not only suitable to the current context in Chile, but are also supported by the best available scientific evidence. Moreover, the implementation of these policies has produced a broad debate involving public institutions and the food industry, with discussions issues ranging from property rights to trade barriers. Despite some differences among stakeholders, a valuable political consensus has been achieved, and several international organizations are eager to evaluate the impact of these pioneer initiatives in Latin America.

Food Programmes (PNAC and PACAM)

These food programmes aim to provide nutritional support and maintain and improve access to healthy and safe food. The National Supplementary Food Programme (PNAC) is a universal programme that provides food to children under 6 years old, pregnant and breastfeeding mothers, and people under 25 diagnosed with inborn errors of metabolism. It aims to promote breastfeeding, and prevent nutritional deficiencies in children and pregnant women. The Supplementary Food Programme for the Elderly (PACAM) distributes micronutrient-fortified foods to older people, with the aim of improving health and preventing and treating nutritional deficiencies.

The Law on Nutritional Composition of Food and its Advertising (Ley 20.606)

Law 20.606 was enacted in 2012, and its associated regulations came into force in 2016. The law requires front-of-package octagonal "High in" labels for food and beverage products that exceed established limits in critical nutrients (calories, sugars, saturated fat, and sodium). It also prohibits the advertising of these products to children under 14, and their sale within schools. It was introduced in three phases with stricter nutrient limits introduced in 2018, and again in 2019.

Elige Vida Sana (Healthy Life) Programme

The Ministry of Health began implementing the "Choose a Healthy Life" (EVS) programme (formerly Vida Sana) in 2015. It is a comprehensive and free treatment that seeks to reduce the risk factors of developing Type 2 Diabetes and Arterial Hypertension, through interventions in eating habits and physical fitness. The programme is implemented by regional health services, and targets children, adolescents, adults and postpartum women. A nutritional diagnosis of overweight or obesity or a diagnosis of pre-diabetes or pre-hypertension is required to be eligible. New programme guidelines were published in 2019, linked below. Taxes on sugar-sweetened beverages: Ley sobre impuesto a las ventas y servicios (Tax law for sales and services)

Law document No.825 updated on September 29th, 2014. Natural or artificial beverages, energized or hypertonic beverages, with syrups or any product replacing it, or that is used to prepare similar beverages, mineral water or water with added artificial colour, flavour or sweetener will have a 10% tax. If the beverage has more than 15g per 240 millilitres or an equivalent portion the tax is 18%.

Pan American Health Organization (PAHO) - Plan of Action for the Prevention of Obesity in Children and Adolescents

This 5-year action plan was signed by PAHO member countries, aiming to halt the rise in obesity among children and adolescents. It calls for a number of policies, including taxes on unhealthy food and drink, labelling and marketing regulation, health promotion, school nutrition improvement, and breastfeeding promotion.

El Sistema Elige Vivir Sano (Choose to Live Healthy System)

Law 20.670 was passed in May 2013, creating the Choose to Live Healthy system (SEVS), a management model consisting of policies. plans and programmes to be developed and implemented by different State Administration bodies "to promote healthy habits and lifestyles to improve people's quality of life and well-being". These habits and lifestyle include healthy eating, physical activity, family life, and outdoor activities, as well as those behaviours and actions that aim to contribute to preventing, reducing, or reversing risk factors and behaviours associated with non-communicable diseases. The Ministry of Social Development and Family is responsible for co-ordinating and running the SEVS, but a range of ministries are involved in co-ordinating SEVS social programmes and campaigns.
